Job Summary:
VitalCore Health Strategies is seeking a skilled Mental Health Professional to join our team at Souza Baranowski Correctional Center. As a key member of our multidisciplinary team, you will provide proactive and collaborative care to patients, working closely with medical staff and community services.
Key Responsibilities:
- Provide crisis stabilization, complete intakes and ongoing evaluations, and offer consultation to in-house and community services.
- Participate in monthly behavioral health calls and collaborate with the medical team to determine comprehensive treatment plans.
- Perform risk assessments of inmates, coordinate care with the interdisciplinary team, and develop discharge plans.
- Contribute to planning and implementing program goals and objectives.
- Prepare documentation, records, and reports as necessary.
- Utilize clinical skills, including prioritizing treatment needs, crisis management, and brief interventions.
Requirements:
- Must be a licensed, masters-level behavioral health clinician with a LCSW, LPC, or equivalent.
- Minimum of one year employment in a social work capacity in a public or private agency or equivalent.
- Must receive a satisfactory background investigation report.
