About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at VITAS Healthcare. As an LPN, you will play a vital role in providing end-of-life care to our patients and their families.

Key Responsibilities
  • Deliver High-Quality Patient Care: Provide skilled bedside nursing care to patients with terminal illnesses, in accordance with the plan of care and the Nurse Practice Act in the state where the VITAS program is located.
  • Evaluate Patient Needs: Assess patient and family needs, and assist in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
  • Collaborate with the Interdisciplinary Team: Work closely with the registered nurse, team manager, and physician to ensure seamless care delivery and effective communication.
  • Document Patient Information: Maintain accurate and detailed documentation of patient information, including clinical notes and interdisciplinary charts.
  • Participate in Continuous Quality Improvement: Contribute to the development and implementation of quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Education and Licensure: Graduate of an approved school of Practical/Vocational nursing and currently licensed to practice nursing in the state where the VITAS program is located.
  • Experience: At least one year of satisfactory nursing experience, with at least one year of experience as an LPN in a hospital on an adult medical surgical unit.
  • Physical Demands: Ability to lift objects in excess of 20 pounds, stand or walk for extended periods, and perform other physical demands of the job.
