Physical Therapist

HTS is seeking a Physical Therapist to join our PRN team at a senior living facility in Indiana. As a Physical Therapist in this position, you will be responsible for conducting evaluations, progress notes, recertifications, and discharges on an as-needed basis. We offer consistent PRN hours if desired.
Responsibilities:- Conduct evaluations, progress notes, recertifications, and discharges as needed
- Develop and implement treatment plans for patients
- Communicate with patients, families, and healthcare professionals
- Maintain accurate and up-to-date patient records
- Flexible scheduling: work only when your availability perfectly aligns with our staffing needs

Requirements:- Applicants must have an active Indiana Physical Therapist license or must have the necessary qualifications to earn Indiana license
- Earn an advanced degree from a Commission on Accreditation in Physical Therapy Education-accredited physical therapist education program
- Pass the state licensure exam
- Fulfill the state's requirements for licensure
