Job Title: Physical Therapist

Job Summary:

We are seeking a highly skilled Physical Therapist to join our team at Wellstar West Georgia Medical Center. As a Physical Therapist, you will be responsible for assessing, planning, and implementing rehabilitative programs to improve mobility, relieve pain, and increase strength in patients suffering from disease or injury.

Responsibilities:

  • Patient Assessment:
    • Collaborate with physicians to establish and evaluate treatment programs, plans of care, goals, and discharge plans for patients.
    • Establish a plan of care in conjunction with patients and/or family members.
    • Develop realistic discharge plans incorporating patient and/or family member goals.
  • Treatment Planning and Provision of Care:
    • Implement physical therapy treatment plans.
    • Select appropriate treatment activities to progress patients towards goals.
    • Revises goals/plan of care with patient/family input.
    • Provide ongoing patient/family education and training.
    • Provide discharge instructions, follow-up, and referral to community resources as appropriate.
  • Documentation:
    • Document evaluations and plans of care according to departmental guidelines.
    • Document each treatment session according to departmental protocols.
    • Complete and submit family education, discharge, and other required documentation within facility and program guidelines.
  • General Requirements:
    • Adhere to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity, and honesty.
    • Embrace diversity and demonstrate the ability to work together.
    • Maintain confidentiality of all patient information.
    • Maintain attendance/punctuality in accordance with system policy and procedure.
    • Adhere to departmental dress code policy, including wearing and displaying ID badge appropriately.
    • Communicate information effectively both verbally and in writing with all team members.
    • Attend and participate in staff meetings, required in-services, process improvement activities, and other meetings as requested.
    • Organize workload to complete responsibilities in an appropriate and timely manner.
    • Utilize materials and equipment in a safe and cost-effective manner.
    • Observe infection prevention precautions.

  • Professional Development:

    • Participate in orientation and training of new employees.
    • Complete all mandatory hospital and/or system mandatory requirements.

Qualifications:

  • Graduate from an accredited physical therapy program.
  • Bachelor's Degree in Physical Therapy.
  • Master's Degree in Physical Therapy.
  • Doctorate in Physical Therapy.
  • Effective communication skills, customer service focused, team oriented, and basic computer skills.
  • Physical Therapist licensure.
  • Basic Life Support certification.
  • BLS - Instructor certification.
  • BLS - Provisional certification.
