Occupational Therapist for Home Health

We are seeking a highly motivated and compassionate Occupational Therapist to join our team of dedicated healthcare professionals. As an Occupational Therapist with FeldCare Connects, you will have the opportunity to make a meaningful difference in the lives of adults of all ages.
About the RoleAs an Occupational Therapist in Home Health, you will be responsible for assessing and evaluating patient care needs, developing personalized treatment plans, and providing interventions to improve functional status and overall wellbeing. You will work closely with patients, families, caregivers, and other healthcare team members to ensure seamless care coordination.
Key Responsibilities- Provide services defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the healthcare team.
- Lead patients in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
- Observe, record, and report patient response to treatment and changes to patient condition.
- Instruct patients, families, caregivers, and healthcare team members in areas of therapy in which they can participate in assisting the patient.
- Occupational Therapist license and registration by the state.
- Completion of an accredited Occupational Therapist program.
- Ability and enthusiasm in working with all ages.
- Bilingual is a plus.
