About the Position

In this role, you will be responsible for assessing patients for transition needs, identifying and addressing barriers to care, and developing effective discharge plans. Your expertise in case management and leadership will enable you to mentor new staff members, provide education and training, and act as a resource to all team members. If you have a Master's degree in Social Work, 3-5 years of experience in social work, and a LCSW or LMSW license, we encourage you to apply.
