Hospice Medical Social Worker

We are seeking a skilled Hospice Medical Social Worker to join our team at Sutter Health. As a key member of our interdisciplinary team, you will play a vital role in providing compassionate care and support to patients and their families during the end-of-life journey.
Key Responsibilities- Assess patients' emotional and practical needs, and develop personalized care plans to address these needs.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
- Provide emotional support and counseling to patients and their loved ones, helping them cope with the emotional and practical challenges of hospice care.
- Develop and maintain relationships with community resources and services to support patients and families.
- Document patient interactions, progress, and outcomes in a timely and accurate manner.
- Masters of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited program.
- 2 years of recent experience in hospice or palliative care, with a focus on social work and patient advocacy.
- Basic Life Support (BLS) certification and valid driver's license.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
