Job Summary


The Community Outreach Specialist serves as a vital member of the community, equipped to deliver education and advocacy to individuals and the broader community.

This role requires an individual who is well-versed in the cultural dynamics and languages prevalent in the community, aiming to provide precise health-related information designed to mitigate recognized health disparities.

It is essential to ensure that information regarding community services is accessible to individuals.

Client engagement may take place in various settings, including clinics, public spaces, and clients' residences, with referrals to the Community Outreach Specialist originating from healthcare providers, care managers, and navigators.

The strategy for the Community Outreach Specialist is formulated in collaboration with the referral source and is grounded in evidence-based practices.


Qualifications


A High School diploma or equivalent is required; an Associate's degree with coursework in Community Health is preferred.

While less than one year of relevant work experience is necessary, a strong desire to assist the community is crucial.

Proficiency in Spanish or Arabic is highly preferred.

A valid driver's license is preferred, with an acceptable driving record and compliance with the qualifications to operate a system vehicle, as well as adherence to all policies outlined in the SJH Fleet Management and Safety Program.
