The ideal candidate will have at least one year of experience working as a LPN and possess excellent clinical skills, strong communication abilities, and a passion for delivering compassionate care.
Job Overview:
The Licensed Practical Nurse will work under the supervision of a Registered Nurse to provide skilled nursing care to patients in accordance with their individualized care plans and physicians' orders. This role requires the ability to observe and assess patient conditions, document care delivered, and coordinate with healthcare teams to ensure seamless patient care.
Key Responsibilities:
- Deliver high-quality patient care in compliance with physician orders and developed care plans.
- Continuously monitor and assess patient conditions, reporting changes to supervisors and physicians as necessary.
- Document all care delivered, including assessments, interventions, and patient responses.
- Collaborate with healthcare teams to develop and update patient care plans.
- Take initiative to prevent risk or reduce risk by recognizing changes in patient needs and implementing appropriate care strategies.
- Maintain accurate records of patient care and progress, adhering to confidentiality standards and professional boundaries.
Requirements:
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Graduate of an accredited school of nursing.
- CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
