Job Summary:

This role is designed for Speech Therapists who require flexibility in their work schedule and are willing to make home visits as part of their responsibilities. As a Home Health Speech Therapist, you will provide independent speech therapy evaluations and treatments to patients in their place of residence, teaching them and their caregivers preventative and rehabilitative procedures. You will also develop and coordinate the plan of care, providing case management and ensuring that patients receive the necessary support to achieve their goals.

Essential Responsibilities:
  • Evaluate patients to determine the nature and extent of speech and language dysfunction, and develop treatment plans to address these issues.
  • Provide instruction and education to patients, their families, and caregivers on speech and language therapy techniques and strategies.
  • Regularly reassess patients to determine the effectiveness of treatment and make adjustments as needed.
  • Initiate discharge planning for speech therapy and ensure that patients receive the necessary support to maintain their progress.
  • Document patient management accurately and timely, in accordance with agency policy and regulatory guidelines.
  • Evaluate the need for interdisciplinary referrals to other healthcare professionals, such as RNs, PTs, OTs, MSWs, and HH Aides.
  • Participate in educational and clinical research activities in speech therapy, and contribute to the development of departmental patient care programs.
  • Supervise the work of Home Health Aides, as required.
Basic Qualifications:
  • Minimum two years of experience in speech therapy, or equivalent experience in a related field.
  • Graduate of an accredited speech therapy curriculum with a bachelor's or master's degree.
  • Speech-Language Pathologist License (California) required at hire.
  • Driver's License (California) required.
  • Basic Life Support required at hire.
  • National Provider Identifier required at hire.
Additional Requirements:
  • Advanced Practice Certification may be required based on operational need.
  • Ability to detect early warning signs of medical complications, including assessments of skin integrity and mentation.
  • Ability to differentiate emergent versus non-emergent patient care status.
  • Commitment to Service Orientation.
  • Effective written and oral communication skills (in English).
  • Good interpersonal skills.
  • Ability to work in a Labor/Management Partnership environment.
