Job Summary

The primary functions of this position include assisting in overseeing the clinical care and treatment of hospice and palliative care patients and families. This involves consulting with patients' attending physicians, providing clinical direction to the Interdisciplinary Group, and ensuring quality care.

Responsibilities
  • Assist in developing and implementing comprehensive care plans for hospice and palliative care patients.
  • Collaborate with the Interdisciplinary Group to provide high-quality patient care.
  • Provide clinical direction and guidance to the Interdisciplinary Group.
  • Consult with patients' attending physicians to ensure continuity of care.
Requirements

To be successful in this role, you must have:

  • A graduate degree from an accredited Baccalaureate School of Nursing.
  • A current State license as a Registered Nurse and as an Advanced Practice Nurse.
  • At least two years of experience as an APN, with clinical care setting preferred.
  • Strong nursing skills, including pharmacology, palliative care, and end-of-life care knowledge.
  • Good interpersonal skills and the ability to work effectively with patients, families, and healthcare teams.
