Job Description:
We are looking for a skilled nutrition consultant who can work independently and as part of an interdisciplinary team. The ideal candidate will have experience in completing MDS assessments, documenting resident information, and participating in care planning processes. Additionally, they will be able to provide oversight to the food service department, ensuring safety, sanitation, and quality of food.

Responsibilities:
* Completion of MDS assessments, Section K, CAA
* Documentation for residents at a high risk for nutritional problems
* Participation in the care planning process
* Collaboration with the Interdisciplinary Team
* Provide oversight to the food service department with routine audit for safety, sanitation, and quality of food

Qualifications:
* Commission on dietetic registration (CDR) credentials
* Licensed (if required by State)
* 2 years of consulting/long term care is preferred, but we will consider many factors
