We are seeking a highly skilled Medication Reconciliation Technician to join our team at Boston Medical Center. As a key member of our Inpatient Pharmacy department, you will play a critical role in ensuring the accuracy and completeness of patient medication histories.
Key Responsibilities- Obtain and verify medication histories from patients and their outpatient pharmacies
- Communicate medication-related issues to healthcare professionals and document findings in the electronic medical record
- Provide exceptional customer service to Boston Medical Center staff and patients
- Maintain confidentiality of patient information and adhere to hospital standards of conduct
- High school diploma or equivalent degree and strong mathematical skills
- Registration as a Pharmacy Technician with the Massachusetts Board of Pharmacy and national pharmacy technician certification
- 4 years of pharmacy technician experience, with at least 1-2 years in outpatient/retail pharmacy
