We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our healthcare team, you will provide high-quality patient care and support to clients in their homes.
Responsibilities- Deliver comprehensive nursing care to clients in their homes, including medication management, wound care, and education on self-care.
- Collaborate with clients, families, and healthcare teams to develop and implement personalized care plans.
- Conduct thorough assessments of clients' physical and emotional needs, and provide supportive care to promote independence and well-being.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care transitions and coordination.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Current valid nursing license in the U.S.
- Graduation from a qualified nursing program
- A competitive hourly rate of $32-$37
