**Job Title:** Travel Nurse RN Home Health Specialist

**Job Summary:** We are seeking a skilled Travel Nurse RN Home Health Specialist to join our team at Supplemental Health Care. As a Travel Nurse RN Home Health Specialist, you will provide high-quality patient care in a home health setting, working closely with our team to deliver exceptional patient outcomes.

**Responsibilities:**

  • Provide skilled nursing care to patients in their homes, focusing on their unique needs and goals.
  • Conduct thorough assessments, develop care plans, and implement interventions to promote patient health and well-being.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress, outcomes, and any changes in their condition.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

**Requirements:**

  • Current RN license in the state of New York
  • Minimum of 1 year of recent home health nursing experience
  • OASIS/Medicare Certified Visits experience
  • Electronic Charting experience
  • Valid U.S. Driver's License
  • American Heart Association BLS
