Speech Language Pathologist Job Description

HealthPRO Heritage is seeking a part-time Speech Language Pathologist to join our team in the Kansas branch. The successful candidate will be responsible for providing high-quality speech and language services to patients in their homes.

Responsibilities:
  • Evaluate patient information to determine the impact on the assessment and treatment process.
  • Conduct thorough assessments to diagnose communicative deficits and equipment needs.
  • Develop and implement individualized treatment programs with achievable functional goals.
  • Comply with evaluation, treatment, and documentation guidelines, maintaining accurate departmental records.
  • Supervise and instruct aides in procedures, ensuring compliance with state law.
  • Enter progress notes in patient records following each treatment session.
  • Revise treatment programs as needed based on patient performance.
  • Document treatment progress in a clear, concise discharge summary.
  • Orient patients to program services and treatment procedures, informing them of potential risks.
  • Work with nursing staff, patients, and families to maximize patient functional abilities.
  • Instruct patients and caregivers on therapeutic procedures to be continued at home.
  • Assist in the referral process, maintaining current knowledge of community resources.
  • Attend and participate in rehabilitation team conferences, providing information on patient progress, problems, or needs.
  • Attend and participate in departmental meetings as required.
  • Access continuing education opportunities to maintain clinical currency.
  • Participate in in-service education sessions and quality improvement processes.
  • Promote program services through formal and informal interactions with the community.
  • Act in compliance with regulatory and professional standards, adhering to facility policies and procedures.
Qualifications:
  • Masters Degree in Speech and Language Therapy.
  • Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) credential.
  • Ongoing continuing education and professional development.
  • Valid state Speech Language Pathology licensure.
