We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at CVMC - Woodridge. As a primary care provider, you will work closely with an interdisciplinary team of healthcare professionals to deliver high-quality care to our long-term residents and short-term post-acute care patients.

The ideal candidate will have excellent clinical skills and the ability to provide continuity of care for patients with chronic and acute illnesses in adults of all ages. You will be responsible for admitting and discharging patients, conducting routine visits, addressing acute issues as they arise, and facilitating specialty and end-of-life care as needed.

This is a 0.5 FTE position with the potential for flexible hours. We offer a dynamic and rewarding work environment where you can make a real difference in the lives of our patients.
