Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Ontario Center for Rehabilitation and Healthcare, a 98-bed Skilled Nursing Facility located in Canandaigua, NY.

Key Responsibilities
  • Record and maintain accurate health details of residents, including vital signs and temperature.
  • Administer medications and injections as needed, ensuring the highest level of patient care.
  • Treat and dress wounds and bedsores, promoting a safe and comfortable environment for residents.
  • Supervise Certified Nursing Assistants (CNAs) and provide guidance and support as needed.
  • Assist residents with daily living activities, such as bathing, dressing, and personal hygiene.
  • Monitor residents' food and liquid intake and output, ensuring their nutritional needs are met.
Requirements
  • Valid NY State LPN license and good standing with the State Registry.
  • Excellent communication and teamwork skills, with the ability to work effectively in a fast-paced environment.
