Job Summary

We are seeking a highly skilled and compassionate Case Manager I/II to join our team at Oklahoma Human Services. As a Case Manager, you will play a vital role in providing services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives.

Key Responsibilities
  • Obtain in-depth social histories and develop community and/or agency resources to meet the consumer's needs
  • Provide monthly monitoring of the program and serve as QIDP
  • Meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines
Requirements

To be successful in this role, you must have a Bachelor's degree in a human service field and relevant experience working directly with individuals with developmental and/or intellectual disabilities. You must also possess a valid driver's license and maintain required car insurance.
