Nurse Practitioner

Glens Falls Hospital is seeking a skilled Registered Nurse to join our team in Cardiac Rehabilitation. As a key member of our interdisciplinary team, you will provide high-quality patient care and support the delivery of exceptional patient outcomes.
Key Responsibilities- Provide direct patient care and develop individualized plans of care in collaboration with the interdisciplinary team.
- Collaborate with physicians and advanced practitioners to provide patient-centered care and ensure seamless transitions across care settings.
- Assume primary accountability for organizing and ensuring nursing care to a select group of assigned patients.
- Develop and implement ambulatory care plans for patients seeking care for health promotion, health maintenance, or health-related situational problems.
- Facilitate care coordination across healthcare settings to maintain continuity of care and ensure patients and populations are directed to the appropriate level of care.
- Bachelor's degree in Nursing (BSN) preferred; new hires will be required to complete their BSN within 5 years of hire.
- One year of nursing experience required.
- Current licensure to practice nursing in NY State without restrictions.
- Basic Life Support (BLS) certification required before completion of orientation.
