Job Title: Licensed Practical Nurse - Care Coordinator

We are seeking a skilled Licensed Practical Nurse to join our team as a Care Coordinator. In this role, you will provide high-quality patient care and coordinate services to ensure seamless transitions and optimal outcomes.

Key Responsibilities:
  • Coordinate and provide Medicare Wellness Visits to eligible patients
  • Utilize Excel spreadsheets to track patients due for Medicare Wellness Visits
  • Work with Medical Assistants and other practice team members to review patient lists, quality measures, and outreach lists
  • Attend Care Management Meetings, Primary Care Collaborative meetings, and other educational sessions as scheduled
  • Follow chronic disease and preventive care guidelines to ensure appropriate plans for completion
Requirements:
  • Must be a licensed LPN in Virginia
  • One year of LPN experience preferred
  • Completion of BLS class and re-certification every two years
  • Ability to make professional nursing decisions up to the scope of practice for licensure
