Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to address the physical, mental, and social needs of patients.
- Collaborate with patients, caregivers, and healthcare providers to ensure seamless care coordination and communication.
- Prescribe medications and treatments as needed, and provide education to patients and caregivers on disease management and wellness strategies.
- Participate in ongoing meetings and quality improvement initiatives to ensure the highest standards of care and patient satisfaction.
- Stay up-to-date on relevant medical and nursing research, technology, and best practices to provide evidence-based care.
- Completion of a Nurse Practitioner accredited program.
- Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- Current, unrestricted RN licensure in the State of Practice.
- Current, unrestricted NP licensure in the State of Practice.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- 2 years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
