**Job Summary:**

We are seeking a dedicated and compassionate Caregiver Specialist to join our team at Supportive Concepts for Families, Inc.

About the Role:

The Caregiver Specialist will be responsible for providing high-quality care and support to individuals with varying needs in a home setting. This includes assisting with daily activities, managing medications, and ensuring the safety and well-being of all residents.

Responsibilities:

  • Consumer Transportation: Provide transportation for medical appointments, community activities, school/day programming, religious services, and family visits.
  • Daily Activities: Assist with meal preparation, activities of daily living, shopping, household cleaning and maintenance, leisure activities, and consumer outcomes.
  • ISP Development and Outcomes: Assist with the development and implementation of Individual Service Plans (ISPs) to ensure positive consumer outcomes.
  • Paperwork and Record-Keeping: Accurately complete required paperwork, including medication logs, daily logs, goal logs, incident reports, sleep/awake charts, weight charts, BM charts, fire drill logs, seizure logs, behavior plan documentation, gas receipts, vehicle logs, and consumer funds.
  • Safety and Well-being: Ensure the safety and well-being of all residents by following operating regulations and guidelines.
  • MEDICATION ADMINISTRATION: Administer consumer medications as prescribed.
  • Team Collaboration: Work closely with Site Supervisors to ensure that all resident needs are met according to their ISP.
  • Training and Competency: Complete required training hours on an annual basis and demonstrate competency-based training as needed.
  • Availability: Be available for shifts twenty-four hours a day, seven days a week as scheduled, and participate in on-call activities and emergency situations as requested.
