Job Summary

Key Responsibilities
  • Manage patient admissions and discharges, ensuring seamless transitions and continuity of care.
  • Delegate nursing rounds and administer medications as prescribed by physicians.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Monitor patient progress, identifying areas for improvement and implementing interventions as needed.
  • Provide education and support to patients and their families, promoting health literacy and self-advocacy.
Requirements
  • Current and active LVN license in the state of practice.
  • One year of experience as a nursing assistant in a skilled rehabilitation, skilled nursing, or Long-Term Care (LTC) environment.
  • Experience with electronic medical records preferred.
