Job Title: Physical Therapist - Patient Care Specialist

Job Summary: We are seeking a skilled Physical Therapist to join our team at Lifepoint Rehabilitation. As a Physical Therapist - Patient Care Specialist, you will be responsible for evaluating patients' conditions, developing treatment plans, and providing hands-on therapy to help them achieve their goals.

Key Responsibilities:

• Evaluate patients' conditions and develop personalized treatment plans
• Provide physical therapy services to patients, including exercises, stretches, and other interventions
• Communicate with patients, families, and healthcare teams to ensure seamless care
• Document patient progress and maintain accurate records
• Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

• Degree from an accredited Physical Therapy program
• Current and unrestricted Physical Therapy license in the state where services are rendered
• Current CPR certification
• Strong organizational and communication skills
