Job Overview

We are seeking a compassionate and skilled Registered Nurse to join our Home Healthcare team in the Twin Cities area. As an Admissions RN, you will have the opportunity to provide patient-centered care in the comfort of patients' own homes.

About the Role:

  • Direct patient care under the supervision of the Director of Nursing or Nursing Supervisor.
  • Create and follow individualized plans of care to meet patients' unique needs.
  • Conduct comprehensive assessments, develop care plans, and implement treatment strategies to ensure optimal patient outcomes.
  • Collaborate with interdisciplinary teams to coordinate patient care and achieve desired results.

Key Responsibilities:

  • Complete admissions or visits for the Home Care team.
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Develop and maintain accurate and up-to-date patient records.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

Requirements:

  • Licensed to practice as an RN in the state of Minnesota.
  • Able to travel within the geographic area serviced by the office.
  • Two years' experience as an RN, preferably in homecare or hospice settings.
  • CPR Certification.

Compensation and Benefits:

  • $76,960 - $89,440 annual salary range.
  • Mileage reimbursement / company car program.
  • Generous PTO, holiday pay, and 401(k) plan.
  • Medical, dental, vision, life, and disability insurance, HSA with company contribution, and more.
  • 100% clinical license reimbursement.
  • CEU reimbursement - up to $400 a year and 2 days of CEU time off.
  • Scrubs allowance.
