Mass General Brigham is seeking a skilled Physical Therapist to join our team in Boston, Massachusetts.
About the Role:This is a unique opportunity to work as a Physical Therapist in a home-based care setting, providing acute-level care to patients in the comfort of their own homes.
Key Responsibilities:- Develop a patient-centered plan of care that incorporates standardized interventions for goal-oriented outcomes.
- Conduct virtual and/or in-person patient evaluations, as needed, and document accurate and timely information to promote real-time updates for the clinical care team.
- Apply standard treatment procedures and provide applicable teaching to patients and caregivers.
- Complete age-appropriate evaluations that reflect patient history, relevant systems review, and appropriate tests and measures, and report to the primary clinician.
- Reassess the plan of treatment daily while admitted to the HH service, modify and communicate as necessary to meet discharge goals.
- Ensure provider orders are current and consistent with the plan of care and documented in the medical record.
- Document complete and timely information according to agency standards.
- Update POC devices as directed and verify payor information on admission in the computer system, adhering to agency standards.
- Meet reportable outcomes that align with patient goals.
- Identify realistic, objective, time-oriented goals for patient performance based on examination findings.
- Develop treatment and discharge plans based on examination findings and patient goals.
- Inform patients of their weekly schedule and any necessary changes.
- Collaborate with the HH Navigator to provide patient identification and coordinate services to meet patient needs.
- Registered with the Board of Registration in Massachusetts.
- Minimum of two (2) years' experience as a licensed Physical Therapist, with homecare and/or acute care experience preferred.
- Strong customer service skills and ability to work with diverse populations.
- Excellent verbal, written, and electronic communication skills.
- Computer proficiency and ability to learn new software, with Epic and Rover experience preferred.
- Ability to prioritize work and adapt to changing situations.
- Travel required within MGB Healthcare at Home geographic area.
