Job Summary

We are seeking a skilled and compassionate Home Health Aide to join our team at Region 1 Inc. As a Home Health Aide, you will provide high-quality patient care and coordinate services to ensure the best possible outcomes for our patients.

Responsibilities
  • Provide direct patient care and coordinate services to meet the unique needs of each patient
  • Assist patients with personal care needs, including bathing, transferring, and ambulation, as assigned by the Registered Nurse
  • Perform incidental household services to prevent or postpone institutionalization, including laundry, meal preparation, and light housekeeping
  • Meet company care standards and actively participate in developing a caseload
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Requirements
  • High school diploma or equivalency
  • Successful completion of an approved training and competency evaluation program
  • Current listing with the Division of Facility Services Registry as an NA-I without substantiated findings
  • One year of patient care and record-keeping experience in an acute care or custodial setting
  • Documented proof of at least 75 certified education hours
  • Valid driver's license
Working Environment

Patient care: 80%, Travel: 15%, Office: 5%

Physical Requirements
  • Ability to walk up and down stairs within patient residences
  • Exposure to blood and body fluids, sharps, and infectious diseases
  • Ability to stoop, kneel, bend, lift up to 50 lbs., push, reach with hands/arms, and smell when providing patient care
