Pharmacist - 2nd Shift Specialist

Polaris Pharmacy Services is seeking a highly skilled and experienced Pharmacist to join our team as a 2nd Shift Specialist. As a key member of our pharmacy team, you will be responsible for ensuring the day-to-day operational duties associated with the dispensing of prescription medication are carried out efficiently and effectively.
Key Responsibilities- Verification and Accuracy: Verify the completeness and accuracy of all new and refill labels and orders, ensuring that all medication packaging and labeling, including emergency drug kits and IV medications, meet the required standards.
- Resident Database Management: Maintain accurate and up-to-date resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication (including OTC and home remedies).
- Controlled Substances Management: Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances, and maintain a perpetual inventory for schedule II drugs.
- Clinical Expertise: Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers.
- Documentation and Communication: Document all communication with physicians, nurses, other members of the healthcare team, and patients/residents and their family members in the medical record.
- Sterile IV Preparation: Prepare and/or supervise the preparation of all sterile IV drug products or solutions.
- Shift Coverage: Provide evening, weekend, and holiday coverage during rotation and as required by management.
- Regulatory Compliance: Ensure adherence to all applicable government regulations, including HIPAA.
- Pharmacist License: Current Pharmacist license active and in good standing in the state of practice.
- Management Experience: Management experience preferred.
- Education: Bachelor's degree or Pharmacy Doctorate from an accredited School of Pharmacy.
- Dispensing Accuracy: Ability to perform accurately and efficiently when dispensing prescription medications.
- Flexibility: Ability to work flexible hours, including evening, weekends, and holidays.
- Technical Skills: Experience using FrameworkLTC and Microsoft Office applications (Word, Excel, Outlook).
- Experience: Minimum of two (2) years recent experience in an acute-care setting or one (1) year experience as a production pharmacist preferred.
- Interpersonal Skills: Excellent interpersonal skills and well-developed verbal and written communication skills with English as the primary language.
- Sitting and Standing: May sit or stand seven (7) to ten (10) hours per day.
- Physical Activity: The employee is occasionally required to sit; climb or balance; and stoop, kneel, bend, crouch, walk, crawl intermittently.
- Lifting and Moving: May lift and/or move up to 50 pounds.
