We are seeking a dedicated and compassionate Registered Nurse to join our team as a Travel Nurse/Home Health Care RN. As a member of our team, you will have the opportunity to make a meaningful impact on patients' lives daily.
About the Role:
- Conduct patient interviews and assessments to address physical and psychosocial health needs.
- Develop individualized care plans and set patient outcomes based on assessment.
- Implement and adjust care plans to ensure safety and optimize patient outcomes.
- Administer prescribed treatments and medications, documenting patient responses.
- Collaborate with physicians and healthcare teams to coordinate care and treatments.
- Educate patients and families about conditions, medications, and post-hospital care.
- Assist in discharge planning and ensure patients receive the appropriate follow-up care.
Requirements:
- Graduate from an accredited School of Nursing; Bachelor's Degree in Nursing preferred.
