This position requires collaboration with the HIV Case Management Team to enhance overall service delivery.

Key Responsibilities:

  • Maintain a thorough understanding of state guidelines for HIV Medical Case Management and comply with established standards.
  • Acquire and apply knowledge regarding HIV/AIDS conditions and treatments to educate clients and their support networks.
  • Provide essential information to clients about HIV, adherence to medication and medical appointments, understanding lab results, and risk reduction methods.
  • Establish partnerships with community organizations to enhance service delivery and combat HIV-related stigma.
  • Assist in the intake process for new clients.
  • Conduct, implement, monitor, and update comprehensive assessments and individual service plans in accordance with health department requirements.
  • Coordinate access to medical, housing, and pharmaceutical services, acting as an advocate for clients within the community health and social services framework.
  • Facilitate enrollment in medical insurance programs, including Medicaid and Medicare, as well as other relevant health initiatives.
  • Provide clients with referrals to a broad spectrum of community resources.
  • Foster and maintain positive relationships with healthcare providers, housing resources, and other social service entities, facilitating referrals as necessary.
  • Contribute as an engaged member of a collaborative team, demonstrating self-direction and the ability to work independently.
  • Participate in committees, task forces, and coalitions within the health district and community, organizing support and educational groups for clients as needed.
  • Maintain accurate and comprehensive client records, documenting all interactions in accordance with program standards.
  • Exhibit excellent written and verbal communication skills, with the ability to perform effectively under pressure.
  • Travel to meet clients and attend relevant meetings and training sessions as required.
  • Ensure medical records are developed and maintained in line with agency policies and procedures.
  • Perform other related duties as assigned.

Required Knowledge, Skills, and Abilities: This role necessitates a solid understanding of case management principles, motivational interviewing, and counseling techniques, along with the ability to work with a diverse clientele facing various medical, psychosocial, and mental health challenges. A foundational knowledge of HIV transmission, acquisition, and treatment is essential. The ability to maintain professional boundaries with clients is critical. Proficiency in Spanish, both written and spoken, is preferred.

Qualifications:

Minimum Requirements: A Bachelor's Degree in social work, psychology, or related behavioral health sciences, accompanied by two years of relevant social service experience. A Master's Degree is preferred.

Additional Requirements: The role requires regular driving, possession of a valid driver's license, and proof of appropriate auto insurance.
