Schedule: Full-Time Days

Shift: Monday to Friday, 8 AM - 5 PM

Role Overview:

The Patient Care Advocate role serves as a vital outreach position within AdventHealth Medical Group, focusing on assessing patients requiring care for various medical conditions identified through data analysis. This position involves conducting chart reviews to determine patient eligibility for outreach, contacting patients to schedule necessary appointments, and ordering lab or imaging studies to facilitate their care. Coordination with primary care providers and their teams is essential to ensure seamless care delivery. The Patient Care Advocate will also play a key role in implementing quality improvement initiatives within the Physician Enterprise, which may include data research, project planning, monitoring quality activities, and providing education to team members and healthcare providers. Additionally, this role will involve educating clinic staff on Quality Measures and supporting the development and maintenance of performance standards and clinical outcomes. A significant aspect of this position includes coordinating and implementing a clinical competency program for non-provider clinical staff in outpatient settings.

  • Provides research support for medical literature to patients, families, or healthcare providers.
  • Engages with payers to negotiate beneficial solutions for patients.
  • Communicates effectively and courteously with patients and colleagues, demonstrating excellent verbal, written, and listening skills.
  • Monitors patient appointments and follow-ups to ensure compliance.
  • Exhibits strong attention to detail, commitment to accuracy, and excellent organizational skills.
  • Designs and implements programs to optimize gap closure processes.

Qualifications:

The expertise and experiences necessary for success in this role include:
  • Minimum of three years of clinical experience.
  • At least three years of experience with Electronic Medical Records (EMR).
  • Active LPN license in the state of Georgia.
