Job Description:
We are seeking a skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will be responsible for assisting in the implementation of the nursing process by performing direct and indirect patient care to assigned patients under the supervision of the Registered Professional Nurse. You will also maintain an orderly and safe patient care environment and unit.
Responsibilities:
- Assist with admission/transfer/discharge of patients, including orienting patients to the hospital environment and providing them with toileting supplies.
- Conduct patient rounds for toileting, positioning, comfort, safety, and personal needs, and answer patient call lights promptly.
- Assemble oxygen equipment and suction set-ups as needed.
- Provide basic hygiene to patients, including bathing, AM/PM care, mouth care, denture care, hair care, nail and foot care, and shaving.
- Prepare patients for meals, including clearing overbed tables and placing them within reach of the patient, positioning the patient correctly for eating or tube feedings, assisting patients with utensils and food items, and feeding patients.
- Perform and/or assist patients with simple treatments, such as removing and reapplying SCD wraps as directed by the RN, and assisting RNs with enema procedures.
- Collect specimens as directed by the RN.
- Inform RNs of any audible clinical alarms, such as IV pumps, feeding pumps, ventilators, bedside monitors, etc.
- Assist with moving, lifting, positioning, and transporting patients.
Requirements:
To qualify, you must have a High School Diploma or equivalent, be a graduate of a Certified Nursing Attendant program or have at least 6 months of relevant experience, hold BLS or Heartsaver CPR/AED certification issued by the American Heart Association (AHA), and possess strong customer service and communication skills.
