RN Navigator

About the Role:

The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care.

This role promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.

Responsibilities:

Requirements:

• Bachelor of Science in Nursing (BSN) required
• (3) years of nursing experience, with one year in specialty required
• Current state RN license required
• BLS Certification required (American Heart Association)
• Certification within specialty, preferred
• Previous cardiology and clinic experience, preferred

Mercy Hospital is an 82 bed, acute care hospital located in the scenic southwestern Colorado town of Durango.
