About the Role: As a Case Management Specialist at Service Coordination, Inc., you will play a vital role in providing quality case management and related services to individuals with Medicaid Home and Community Based services.

Your primary responsibilities will include advocating, arranging, and monitoring services, identifying supports, and establishing positive relationships with the people we support and other stakeholders. You will also facilitate the creation of person-centered plans, monitor progress, and ensure that services meet state-mandated standards.

We offer a competitive salary of $62,217 - $77,160 annually, depending on experience, plus an array of benefits, including tuition reimbursement, student loan repayment, medical, dental, and vision plans, 100% employer-paid short-term and long-term disability, life insurance, 403(b) match, and paid training.

**What We're Looking For:**
  • A Bachelor's or Master's degree in a human service field.
  • Experience in working with people with disabilities and person-centered planning.
