Key Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients and their families
• Develop and adjust patient care plans as needed
• Utilize health assessment data to establish nursing diagnoses
• Administer prescribed medications and treatments
• Provide counseling to patients and families to address nursing and related needs
• Maintain accurate clinical documentation
• Collaborate with physicians and participate in interdisciplinary meetings to address the medical and emotional needs of patients and their caregivers
• Ensure adherence to all state and federal regulations
• Perform additional duties as assigned

Qualifications

• Graduate of an accredited nursing program with current licensure in the state of practice
• Minimum of 1 year of nursing experience required; 3+ years preferred
• Experience in hospice or a similar healthcare setting is preferred
• Understanding of the hospice philosophy of care
• Commitment to excellence in clinical practice and documentation
