**Job Summary:**
In this pivotal position, you will serve as a member of our Street Outreach Team, conducting outreach to homeless individuals at LIRR stations across Nassau County. Your primary responsibility will be to assess clients' needs, document strengths and goals, and facilitate access to essential services. You will maintain effective communication with clients, healthcare providers, family members, and other stakeholders to ensure seamless coordination of care.

**Key Responsibilities:**
• Conduct street outreach to homeless individuals, assessing their needs and documenting strengths and goals
• Maintain accurate records, statistics, and reports as per policy and procedure
• Collaborate with clients, healthcare providers, family members, and other stakeholders to coordinate care and ensure timely access to services
• Participate in regular meetings with the team to discuss client status, update plans, and review outcomes
• Act as a resource and consultant to team members on psychosocial and substance abuse issues
• Provide crisis intervention and follow-up services as needed

**Requirements:**
• Bachelor's Degree in Social Work, Psychology, or a related field
• 1-2 years of experience in human services, with a focus on direct client care
• Strong knowledge of Medicaid, Social Security, and other entitlement programs
• Excellent interpersonal skills and ability to work effectively in a team environment
• Valid driver's license and reliable transportation
• Ability to cultivate a culture of inclusion and respect within the workplace
