**Job Summary**

As a Licensed Practical Nurse, you will be responsible for delivering high-quality nursing care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our residents.

**Responsibilities**

* Provide direct nursing care to residents, including administering medications, performing treatments, and monitoring vital signs
* Develop and implement individualized care plans, in collaboration with the interdisciplinary team
* Conduct thorough assessments of residents' physical and emotional needs
* Communicate effectively with residents, families, and healthcare team members
* Participate in quality improvement initiatives and contribute to a culture of excellence

**Requirements**

* Active, and in good standing, Licensed Practical Nurse (LPN) license in the state of practice
* Prior experience as a Licensed Practical Nurse (LPN) in a long-term care setting preferred
* Excellent communication and interpersonal skills
* Ability to work effectively in a fast-paced environment
