Senior Resident Care Coordinator

About the Position
">This senior role requires a highly skilled and experienced professional to oversee the development, completion, and updates of resident assessments and care plans. The successful candidate will work closely with multiple departments to ensure compliance with state regulations and community policies.
">Key Responsibilities:

- Coordinate and facilitate care plan conferences">
- Monitor and guide the completion of PPS, OBRA, and assessment reports">
- Maintain and update written policies and procedures related to resident assessments and care plans">
- Transmit completed MDS reports to relevant parties on a weekly basis">
- Assess prospective residents in conjunction with other team members">
- Perform administrative duties as needed, including completing medical forms and evaluations">
- Develop and maintain a quality assurance program for resident assessments and care plans">
Requirements
">- ">
- Bachelor's degree in Nursing or related field">
- Current, unencumbered RN license in the state of operation">
- At least two years of managerial experience in a hospital or nursing care setting">
- Schedule flexibility, with ability to rotate on-call">
- Excellent problem-solving skills and knowledge of medical practices and procedures">
- Strong communication and customer service skills">
Compensation and Benefits
">We offer a competitive salary range of $45,000 - $55,000 per year, based on experience, as well as a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
