Job Title: Care Manager

At Corner Health Center, we are seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in supporting patients who require additional resources to complement their care.

Job Summary

The Care Manager will serve as a liaison between patients, medical providers, and community resources to ensure seamless care coordination. This role requires excellent communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Assist patients with applications and referrals for public benefits, including Medicaid, Marketplace, cash assistance, food stamps, WIC, and state emergency relief.
  • Connect patients with community resources, such as basic needs, childcare, clothing, food, educational, and shelter services.
  • Maintain an updated list of community resources and foster relationships with agencies to which we refer frequently.
  • Facilitate communication and care among medical, behavioral health, and other providers at CHC to help meet the needs of each individual patient.
  • Support CHC health education and outreach programs and services by providing care management-related activities.
  • Refer patients to outside providers for specialty care, such as physical therapy, early intervention, hearing tests, substance use, and speech therapy.
  • Meet with patients as needed for transitioning their physical and mental health care as they age out of our services.
  • Keep accurate, updated case notes and timely billing for all services.
  • Collaborate with nursing staff to contact insurance carriers to determine a patient's plan coverage.
  • Collaborate with nursing staff to ensure appropriate follow-up for CHC patients discharged from the hospital and follow up for missed appointments.
  • Provide supplemental health education and outreach as needed.
  • Keep track of patients' records, especially electronic records, and maintain current skills with technology, software, and security measures.
  • Oversee operations of the Corner Store, including volunteer recruitment, training, and supervision.
  • Attend trainings/continuing education related to job responsibilities.
  • Participate in clinic and program meetings relevant to job duties.
  • Assist in clinic as appropriate, including as Social Worker of the Day, supporting in crisis management and monitoring the SWOD inbox for referrals.
  • May represent The Corner at community events.
  • Provide other services at the request of the Clinic Manager.
  • Occasional evenings or weekends required for special events.
Requirements
  • Bachelor's degree in Psychology, Social Work, Health Education, Nursing, or certification in a related area is required.
  • Master's degree in Social Work with licensure in the State of Michigan is highly preferred.
  • 3+ years of experience providing care management, care coordination, social work, or other community resource navigation is highly preferred.
  • Commitment to the agency's mission and DEI initiatives.
  • Provide services in accordance with the MDHHS CAHC Minimum Program Requirements and grant guidelines.

  • Ability to stay calm in difficult and emotional situations.
  • Proficient in Microsoft Office Suite or related software.
Salary Range

$50-60K
