Optum Home & Community Care is seeking a highly skilled Advanced Practice Clinician to join our team. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.
Key Responsibilities- Collaborate with Interdisciplinary Team: Work closely with our team of healthcare professionals, including physicians, nurses, and social workers, to develop and implement comprehensive care plans for our patients.
- Conduct Comprehensive Assessments: Perform initial and bi-annual assessments of patients to identify their unique needs and develop personalized care plans.
- Prescribe Appropriate Diagnostics and Interventions: Use your expertise to prescribe the necessary diagnostics and interventions to prevent unnecessary hospitalizations and improve patient outcomes.
- Consult with Hospital Physicians: Collaborate with hospital physicians to ensure seamless transitions of care and optimal patient outcomes.
- Participate in Quality Improvement Initiatives: Contribute to our quality improvement efforts by implementing evidence-based practices and participating in regular quality improvement meetings.
- Stay Current with Medical and Nursing Research: Stay up-to-date with the latest medical and nursing research, technology, and best practices to ensure you are providing the highest level of care.
- Completion of Advanced Practice Clinician Program: Must have completed an accredited program for Advanced Practice Clinicians.
- Current State RN License: Must have a current state RN license.
- Current NP or PA License: Must have a current NP or PA license.
- Certification through AANP, ANCC, or NCCPA: Must be certified through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Proven Knowledge of Long-Term Care: Must have proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Access to Reliable Transportation: Must have access to reliable transportation and comply with our Motor Vehicle Safety policy.
- 2+ Years of Clinical Experience: Preferred candidates will have 2+ years of clinical experience.
- Knowledge of CPT, ICD-9, and HCPC Codes: Knowledge of CPT, ICD-9, and HCPC codes is preferred.
- Knowledge of Clinical Standards of Care: Knowledge of clinical standards of care is preferred.
- Awareness of NCQA Requirements, CMS Guidelines, Medicaid/Medicare Contracts, and Benefit Systems: Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems is preferred.
