About the Role

We are seeking a skilled Physical Therapist to join our Home and Community Program team at BrightSpring Health Services. As a Physical Therapist, you will play a critical role in providing functional rehabilitation services to individuals with brain injuries, spinal cord injuries, or stroke/CVA in their natural home and community environment.

Key Responsibilities

  • Perform comprehensive evaluations and develop individualized treatment plans to promote functional capabilities and independence.
  • Regularly reassess treatment plans and make adjustments as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and collaboration.
  • Provide education and resources to patients and families to support their recovery and independence.
  • Collaborate with other disciplines to ensure comprehensive care and enhance patient outcomes.

Requirements

  • Master's or Doctoral degree in Physical Therapy from an accredited program.
  • Current, unrestricted license as a Physical Therapist in the state where practicing.
  • Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
  • Minimum of one year's work experience as a Physical Therapist, preferably in a neuro rehabilitation setting.
  • Strong knowledge of rehabilitation techniques related to complex neurological injuries.
  • Excellent communication and interpersonal skills.
  • Ability to lift 50 pounds and perform physical demands of the job.
