Senior Primary Care Physician

About the Role
CenterWell Senior Primary Care is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a Field-Based Nurse Practitioner, you will be responsible for providing proactive, preventive care to seniors in their homes. Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being.
Responsibilities
- Provide health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions.
- Prescribe medication, examine patients, diagnose illnesses, and provide treatment.
- Advises executives to develop functional strategies on matters of significance.
- Exercises independent judgment and decision making on complex issues regarding job duties and related tasks.
Requirements
- Valid state driver's license, clean driving record, and proof of auto insurance.
- Bachelor's degree.
- Minimum 2 years clinical experience.
- Must have an active/unrestricted state license, DEA license, and board certification.
- Senior patient care experience.
- Active BLS through the AHA (American Heart Association) or ARC (American Red Cross).
- Strong technical experience.
- 2 or more years of project leadership experience.
Preferred Qualifications
- Master's in science of nursing.
- House call experience.
- Urgent care, primary care, family practice, or internal medicine experience.
- Value-based care knowledge and/or experience.
- HEDIS quality metrics knowledge and/or experience.
