Job Title: Registered Nurse (RN)

Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at IntelyCare. As a RN, you will have the opportunity to provide high-quality patient care in post-acute care facilities, working collaboratively with medical staff and residents to deliver the best possible outcomes.

Responsibilities:

  • Provide direct patient care, including administering medications, performing treatments, and monitoring medical equipment.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each resident.
  • Collaborate with medical staff and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Conduct thorough assessments and evaluations to identify potential health risks and develop strategies for prevention and intervention.
  • Communicate effectively with residents, families, and healthcare teams to ensure that all parties are informed and engaged in the care process.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
  • Contribute to a positive and supportive work environment, promoting a culture of respect, empathy, and teamwork.

Requirements:

  • Graduation from an accredited nursing program.
  • At least 4 months of post-acute care or in-patient hospital experience.
  • Current RN licensure and certification in good standing.
