Key Responsibilities- Provide clinical pharmacy services to patients in an ambulatory setting, including medication therapy management, patient education, and counseling.
- Collaborate with healthcare teams to develop and implement patient care plans that promote optimal health outcomes.
- Conduct medication use evaluations and provide recommendations for improvement.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Maintain accurate and up-to-date patient records, including medication lists and treatment plans.
- Doctor of Pharmacy degree from an accredited institution.
- Current licensure as a pharmacist in the state of California.
- Board certification in a relevant specialty (e.g., BCACP, BCPS).
- Experience in ambulatory care pharmacy practice, preferably in a hospital or health system setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, healthcare teams, and other stakeholders.
This position requires the ability to work in a fast-paced environment, with frequent interruptions and deadlines. The ideal candidate will be able to prioritize tasks effectively, manage multiple responsibilities, and maintain a high level of productivity in a dynamic setting.
