Support Services Manager

Communitas, Inc. seeks a highly skilled Care Coordinator to join our team in Wakefield, MA. As a key member of our Day Program Services, you will work closely with individuals with developmental disabilities, their families, and interdisciplinary teams to ensure comprehensive support and coordination of services.
About the Position- Job Summary: The Care Coordinator will be responsible for building and maintaining professional relationships with individuals on their caseload, their families, and members of each individual's interdisciplinary team to ensure ongoing coordination of services.
Key Responsibilities:
- Work collaboratively with other departments, individuals, families, residences, and funding sources to ensure that services meet individual needs and promote individual choices.
- Coordinate or complete assessments/evaluations to determine support levels and support planning, as well as conduct re-evaluations on an as-needed basis.
- Monitor follow-through and effectiveness of support plans with department staff and interdisciplinary team in cooperation with the department manager, providing guidance to department staff in completing required documentation.
- Plan for and participate in planning meetings (team meetings, DHSP, ISP) actively advocating for individuals' needs and contributing to overall goals of the department and individuals supported.
- Provide personal care and assistance to individuals consistent with individual needs as they strive to reach their goals, providing coverage for groups as needed.
Requirements
- A high school diploma is required; a BA degree in Human Services or a related field is preferred.
- At least one year of experience supporting adults with developmental disabilities is preferred.
- A passion and dedication to supporting people with developmental disabilities is essential.
- A valid driver's license, reliable transportation, and an acceptable driving record are necessary.
- The ability to pass company background and reference checks is required.

- The estimated annual salary for this role is $45,760, based on a 40-hour workweek and $22.00 per hour.
