Primary Care Provider for HouseCalls Program in NY

Optum is seeking a skilled Primary Care Provider to join our innovative HouseCalls program in Columbia and Greene Counties, New York. As a clinician-led care organization, we're dedicated to creating seamless health journeys for patients across various care settings.
Key Responsibilities- Conduct comprehensive in-home assessments, evaluating past medical history, reviewing systems, reconciling medications, taking vital signs, and performing physical exams.
- Implement evidence-based practice screenings, including point-of-care testing when necessary.
- IDentify diagnoses to inform care management and active medical management.
- Communicate findings with members' primary care physicians to address potential gaps in care.
- Educate members on disease processes, medication management, and adherence.
To succeed in this role, you must possess:
- An active NP or PA license or be able to obtain one by start date, with an unencumbered licensure free of any open/unresolved disciplinary actions.
- National certification from ANCC or AANP in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or the ability to obtain certification and/or licensure in the state of assignment by start date.
- The capacity to complete the job's physical requirements, including lifting 30 pounds, navigating stairs, and sitting, standing, and kneeling for physical assessments.
- Reliable transportation for home visits.
- Maintainability of additional licensure in other geographic areas.
- At least 1 year of clinical experience in their highest level of education, clinical setting.
- Gerontology, cardiology, internal medicine, or endocrinology experience.
- Home healthcare or home visit experience.
- Ability to travel throughout the state, beyond the assigned region, up to 5% of the time, for which eligible employees may receive additional compensation incentives.
This position offers a competitive salary range of $88,000 to $173,200 annually, based on factors such as local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws applicable. In addition to your salary, you'll have access to a comprehensive benefits package, incentive programs, equity stock purchase, and a 401(k) contribution (all subject to eligibility requirements).
No on-call shifts, weekends, or holidays are required in this role, allowing for flexible scheduling and achieving a better work-life balance.
