Our Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.

As a Home Health Risk Assessment Specialist, you will have the opportunity to make a direct impact on the health and well-being of others, leveraging your expertise and compassion to drive meaningful outcomes.

Responsibilities
  • Conduct thorough adult and geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
  • Cultivate relationships with Primary Care Physicians (PCPs) for patient education, provide follow-up, and ensure seamless communication.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Hold current BLS, ACLS, or CPR certification.
  • Minimum 1 year of experience as a Nurse Practitioner/Physician Assistant, with ability to travel in-state or out-of-state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • A positive and outgoing personality, with ability to thrive in a fast-paced environment and adapt to frequent change.
