Job Summary

Telecare Corporation is seeking a skilled Social Worker to join our team at the Riverside Psychiatric Health Facility in Indio. As a Social Worker, you will provide clinical and casework services to members served and natural supports, working collaboratively with multidisciplinary treatment teams to design, implement, and evaluate recovery plans.

Key Responsibilities
  • Provide safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care
  • Work with members served and multidisciplinary treatment teams to design, implement, and evaluate recovery plans
  • Establish and maintain a therapeutic relationship with members served, ensuring provision of needed clinical services and coordination of ongoing treatment within the program and with outside agencies
  • Document all treatment provided in a thorough and consistent manner
  • Collaborate with other staff in the discharge planning process and make linkages to community services for members served
Requirements
  • A Master's Degree in Social Work from an accredited graduate school and working towards licensure and registered with the Board of Behavioral Science
  • One (1) year of direct service experience with individuals with severe mental illness in an inpatient or outpatient setting
  • Criminal justice clearance and education verification
  • Must be at least 18 years of age
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
Preferred Qualifications
  • Previous experience with population to be served and/or work in a similar program setting
Skills
  • Ability to complete social histories, risk assessments, mental health status exams, and plans of care
  • Ability to work collaboratively with physicians, nurses, and rehabilitation therapists
  • Ability to form a therapeutic alliance with members served and families
  • Sufficient medical knowledge to assess the needs of members served
  • Skills in conducting group therapy
  • Ability to advocate for the members served in other health delivery systems
Physical Demands

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to sit, stand, walk, bend, push, pull, squat, kneel, and lift and carry items weighing 25 pounds or less as well as to frequently reach, twist, and do simple and power grasping.

The position requires manual deviation, repetition, and dexterity.
