Job Summary

The successful candidate will be responsible for delivering exceptional resident care, working collaboratively with our multidisciplinary team to ensure the best possible outcomes. This role requires a strong commitment to patient-centered care, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities
  • Provide personalized care to residents, including bathing, dressing, and grooming, in accordance with their established care plans.
  • Assist residents with daily activities, such as feeding, toileting, and mobility, as needed.
  • Monitor and report any changes in resident condition to the Charge Nurse or other healthcare professionals.
  • Participate in inter-disciplinary team meetings to discuss resident care plans and share knowledge and expertise.
  • Maintain accurate and up-to-date records of resident care, including medication administration and vital signs.
Requirements
  • Graduate of an accredited high school or GED preferred.
  • Active Certified Nursing Assistant Certification, with successful completion of a State-approved training program and necessary examination.
  • Ability to work effectively in a team environment and communicate with residents, family members, and healthcare professionals.
  • Physical stamina to perform duties in a fast-paced environment, including standing, walking, and lifting.
